1. My Frame of Health, all advice and information provided is not to be a substitute for professional medical advice, diagnosis or treatment. It is NOT the purpose of this consultation/training to dispense specific medical advice or any other advice that would be relied upon by any individual for medical purposes. All clients should seek a doctor’s advice for starting any program that involves lifestyle changes.
Reliance of all advice and information is solely done at your own risk and discretion.
2. I recognize that My Frame of Health is not responsible for any results that I may be seeking. I recognize that all information is for learning purposes and is to be used at my discretion.
3. Privacy. Information collected is absolutely confidential and will not be used by another party. All information gathered is used by the consultant for use in preparing individual recommendations.
4. All information and materials are personalized and shall not be used by any one but the client. All materials, advice, information are not to be duplicated, sold or used for anything other than learning materials between client and My Frame of Health. You do confirm that all information you provide is not false and only pertains to yourself.
5. My Frame of Health is not in any way liable for any injury, illness or misuse that involves the client and/or the client’s property. There is no liability of the consultant of any injury or mishap during any session in any location or meeting facility.
6. (Does not pretain to free services) Payments are to be provided in full before services are rendered. There is no refund of funds after payment is made.
There is a $25 US charge for returned checks.
Any rescheduling of appointments should be made two days prior.
